Where: All-purpose room or Auditorium Porch (weather dependent)
For: Parents of all WFS students, WFS students 12 years and older, accompanied by a parent, WFS teachers
Needed for participation:
- RSVP by 9/15 by 3pm to: [email protected]
- A copy of your insurance card or $39.99 in cash
- A completed screening form, which will be sent home in your child's backpack
- A short sleeve shirt to be worn on the day of the clinic
